HOW TO ACCESS Simple Practice Patient Portal

When you become a patient, you'll receive a Welcome email with a link to complete your paperwork.

Click the link in the email to go to the portal sign-in page. Enter your email and temporary passcode.

After logging in, create a permanent password for future access.

Click Get Started to review and agree to Practice Documents and complete any required forms.

Once you submit your paperwork, you'll be taken to the main portal page, where you can view your completed documents.
